Your logbook records every clinical procedure you are assessed on. You do not
enter scores yourself — a clinical instructor does that after watching you perform
the procedure. Your job is to keep your profile correct, know what is outstanding,
and present your QR card when you are assessed.
1. Complete your profile first
On first sign-in you are asked for your student ID, IC number, phone and intake.
You cannot reach the rest of the system until this is saved. Add a photo if you can —
it appears on your QR card and helps instructors confirm they have the right student.
You cannot set your own semester. The faculty administrator places you,
and moves the whole cohort forward at the end of each session. If your semester looks
wrong, tell your lecturer.
2. Your dashboard
The dashboard answers one question: what do I need to do next?
- Needs your attention — procedures whose most recent attempt did not
pass, with the instructor's remark. This is your to-do list. Click one to open that
semester's logbook.
- Overall / Passed / Need retry / Not assessed — your position across the
whole programme.
- Current semester — progress bars for compulsory and optional
procedures, with a Continue button into the logbook.
- Recent feedback — the last five assessments with scores and remarks.
3. Your logbook
My Logbook lists every semester with its progress. Open one to see its
procedures, grouped as they are in the syllabus.
- Use the search box to find a procedure by name instead of scrolling.
- Filter by Passed, Retry or Not assessed
to see only what matters right now.
- Each group header shows how many of its procedures you have passed.
- Click the icon on a row to see every attempt at that
procedure — date, score, who assessed you, and their remark.
How scoring works

A procedure counts as passed at {{ config('assessment.pass_score') }} or above.
Only your most recent attempt counts — so if you scored 2 and later
scored 5, the procedure is passed and leaves your to-do list.
4. Getting assessed — your QR card
Open My QR Card from the dashboard. A clinical instructor scans it to open
your record straight away, without searching for you.
- Show it on your phone, or press Print card for a lanyard-sized badge.
- If the camera will not read it, the instructor can search for your student ID instead —
it is printed on the card for that reason.
5. Your clinical performance report
At the end of a placement your instructor completes a report covering attendance, competency
and remarks. Once submitted it appears on your logbook semester page and can be downloaded
as a PDF. If no report exists yet, there is nothing to open — ask your instructor.
